One Stop Pharmacy Privacy Notice
One Stop Pharmacy may receive your personal data from a number of different sources which may include:
• Yourself
• Your representative e.g., guardian
• Your clinical team e.g., clinicians and nurses
• Referring hospital
• Social care services
The personal data we may process includes your:
• Name
• Date of birth
• Gender
• Contact details (e.g., address, email and contact number)
• NHS number
• Relevant medical conditions
• Prescription medicine names and their directions for use which includes the dose and frequency
• Notes of contact made with you directly, your representative or with your clinic
We use your personal data to enable us to provide pharmacy and clinical homecare services to you. We may process this data for the following purposes:
• Your care – providing pharmacy services and care to you and, as appropriate, sharing your information with your clinical and homecare teams
• Our payments – sharing your information with the NHS Business Services Authority, others in the wider NHS, and sometimes Local Authorities, and only limited information to those external to the NHS who negotiate and check the accuracy of our payments; and,
• Drug safety – sharing certain limited information with pharmaceutical companies to ensure we are compliant with adverse events reporting or side effects relating to your medication that has been reported to One Stop Pharmacy
• Deliveries – sharing information such as your name, delivery address, contact number with our own internal logistics team and/or logistics partners that deliver your medication and ancillaries on behalf of One Stop Pharmacy
• Management – sharing only limited information with the NHS Business Services Authority and others in the wider NHS, and sometimes Local Authorities; as well as those external to the NHS who ensure we maintain appropriate professional and service standards and that your declarations and ours are accurate.
• Patient Support Programmes (PSP) – to help you get the best from your medicine. If you’re eligible for a PSP, then you will be asked if you’re content to participate and give consent to sharing of your personal data prior to the commencement of services.
We hold your information for as long as advised by the NHS and our regulatory bodies to ensure we are compliant with our legal and regulatory obligations. You have a right to a copy of the information we hold about you, generally without charge. You may seek to correct any inaccurate information.
We process your personal data so we can provide pharmacy and clinical homecare services to you, in collaboration with your clinical team, to ensure you get the best out of your treatment for your condition.
A pharmacist is responsible for the confidentiality of your information.
You may object to us holding your information. You may also lodge a complaint with the Information Commissioner's Office, Wycliffe House, Water Lane, Wilmslow, Cheshire SK9 5AF. Please ask if you want more information.
